Tectonic and therapeutic keratoplasties part 2: fungal keratitis can be very challenging to treat. Those aggressive infections which don’t respond to intensive anti-fungal treatment and continue to spread, threatening to reach the sclera, need a therapeutic keratoplasty, frequently a large size one, including some scleral margin. This is only the first step to control the infection; postoperative medical management to avoid recurrences is key and often challenging.
